Qualifications :

Required :

• Associate degree or higher
• 1+ years of experience in ANY of the following :
• Electronic Medical Record (EMR)
• Other EMR
• TruChart EHR
• 1 plus years of experience in ANY of the following:
• Acute Care
• Rehabilitative Setting
• Valid Physical Therapy Assistant in the state of Illinois (IL)
• Valid Regular Driver License (DL)
• Degree in Physical Therapy

Preferred :

• 1 plus years of experience in Home Health

• Collaborating to provide patient care plans.
• Supporting patients in the area.
• Demonstrating skills in time management and technology use.

Key Responsibilities :

• Collaborate to provide management and delivery of patient care plans.
• Support patients in Springfield, IL, and surrounding areas.
• Consistently meet critical deadlines for charting
• Self-manage time and schedule.

Schedule and Shift Details :

• Traditional Monday through Friday from 8:00 a.m. - 5:00 p.m.

• Local travel
